Health Function Ltd respects your privacy. This policy explains what personal information we collect through the healthfunction.co.nz website, why we collect it, and your rights under the Privacy Act 2020.
This policy covers the website only. Health information held about registered patients is covered by the Patient Rights, Privacy and Consent to Treatment document, which also explains your rights under the Health Information Privacy Code 2020.
What we collect
Through this website we collect only what you choose to give us.
When you apply to register, the application form collects your name, date of birth, contact details and physical address, along with your reason for making contact, other practitioners you have seen about it, your diet, and how you heard about the practice. Some of those answers are health information, and we treat them accordingly.
If you use the contact form, it collects your name, email address, subject and message, and whether you would like to receive occasional newsletters. If you contact us by email, we hold that correspondence.
This website uses no analytics and no tracking cookies. If that changes, this policy will be updated before it does.
Why we collect it
Information from the application form is used only to assess your application and to contact you about it. If you are accepted, it becomes the beginning of your clinical record. It is not used for marketing and is not shared for advertising.
How it is processed and stored
The application form and the contact form are processed by Formspark, whose servers are located outside New Zealand, and delivered to us by email. The website is hosted by Cloudflare. These providers act as our processors and hold data on our behalf under their own security and privacy terms.
Where an application is accepted, the information you have given is entered by hand into Indici, the practice’s clinical record system, and becomes part of your clinical record.
How long we keep it
An application is held on file for twelve months. If a place has not been offered in that time it is deleted, and you are welcome to apply again.
Once an application has been accepted, the information forms part of your clinical record and is kept for ten years from the date you were last seen, as required of health records in New Zealand.
Who we share it with
We do not sell your personal information and we do not share it for advertising. It is shared only with the service providers described above, as necessary to operate this website, and where required by law.
Your rights
Under the Privacy Act 2020 you may:
- request access to the personal information we hold about you
- request correction of it
- ask any question about this policy
